New Hampshire Colonoscopy Registry

Concord Endoscopy Center and the physicians performing procedures at the center participate in the New Hampshire Colonoscopy Registry. The New Hampshire Colonoscopy Registry is funded by the National Cancer Institute and is administered by Dr. Allen Dietrich and Dr. Lynn Butterly at Dartmouth Hitchcock Hospital. Many hospitals and endoscopy centers throughout New Hampshire participate in the study.

The primary goal of the study is to learn about how medical, environmental and inherited factors affect the incidence of colon cancer in New Hampshire. Although there may not be any direct benefits to you, the information you provide may benefit others in the future as the incidence of polyps and colon cancer are studied.

Your personal information will be held in the strictest of confidence at Concord Endoscopy Center and Dartmouth Hitchcock Hospital. De-identified statistical information (aggregate data) will be sent to the National Cancer Institute. Your personal information is protected; however the National Cancer Institute may use the statistical information to study the incidence and affects of colon polyps or colon cancer. Your participation is entirely voluntary. If you participate and change your mind after you leave the center, you may withdraw your information by contacting Dr. Lynn Butterly in writing at Dartmouth Hitchcock Hospital.

You will be given information about the study and asked to read and sign the consent to participate when you arrive at the Concord Endoscopy Center. You will be asked to complete a short survey form at the time of the procedure and an additional survey will be mailed to you in about 30 days after your colonoscopy. If you have any questions about the survey, please ask your admitting nurse or physician at the center.
